2008 FALL BALL REGISTRATION

(Volunteer checks may be picked up this Sat. 8/2 between 9:30am and noon)

PLACE:     NHLL COMPLEX -         Okerson Road

DATES:     SAT. AUG. 2^ND                9:30AM - NOON (Refund pickup)

                  THU. AUG. 7^TH                 6:30PM – 8:30PM

                  SAT. AUG. 9^TH               9:30AM - NOON

                  THU. AUG. 14^TH             6:30PM – 8:30PM

                  SAT. AUG. 16^TH                9:30AM – NOON

Registration fee    $50 5 thru 7 year old

                              $85 older than 7 years old

Please have all required forms completed before you arrive. These include:

-      registration form

Complete only if child didn’t participate in spring ball

-      medical release form

-      NJS code of conduct

-      volunteer form
