required forms due at registration:

Parent/Guardian NJ State Code of Conduct Form
New players to the League are required to provide a copy of their Birth
Certificate as proof of age. If you have a lost or
misplaced Birth Certificate, click here for info:

As mandated by Township ordinance, every
participant's parent and/or guardian is required to sign the State of
New Jersey Model
Athletic Code of Conduct. This includes all Board Members and
Volunteers regardless of whether or not they have a child in the League,
no exceptions! .
